Condition Assessment

Nurses provide in-home clinical perspective for health conditions like Pain, Mobility or Swallowing. During a condition assessment the nurse documents condition changes to support ongoing clinical analysis and referral.

Condition Assessments include:

  • Monitoring, assessment and documentation of a changing health condition
  • Documenting impacts on quality of life to substantiate care needs
  • Provides advice on preventative health
  • Referral to emergency services when indicated
Assessment is What is happening. Diagnosis is Why it is happening.  Nursing assessments monitor what’s changing with someone’s health condition,  how serious it is and what general steps to take next. Call 911 right away for emergencies including

:

  • Chest pain or tightness
  • Choking or breathing difficulties
  • Loss of consciousness
  • Signs of a stroke
  • Severe burns
  • Convulsions that are not stopping
  • A drowning
  • A severe allergic reaction
  • A serious head injury
  • Major bleeding
  • Imminent childbirth
