Condition Assessment
Nurses provide in-home clinical perspective for health conditions like Pain, Mobility or Swallowing. During a condition assessment the nurse documents condition changes to support ongoing clinical analysis and referral.Condition Assessments include:
- Monitoring, assessment and documentation of a changing health condition
- Documenting impacts on quality of life to substantiate care needs
- Provides advice on preventative health
- Referral to emergency services when indicated
Call 911 right away for emergencies including
:
- Chest pain or tightness
- Choking or breathing difficulties
- Loss of consciousness
- Signs of a stroke
- Severe burns
- Convulsions that are not stopping
- A drowning
- A severe allergic reaction
- A serious head injury
- Major bleeding
- Imminent childbirth
