Nurses provide in-home wound care supporting the healing of post-op incisions, cuts, skin tears, burns, ulcers and other types of wound. Below is a basic explanation of the difference between Creating a Wound Care Plan vs Following a Wound Care Plan. 

Creating a Wound Care Plan (RN Only)

  • Understanding the client history and actions leading up to the wound
  • Inspecting the wound
  • Assessment for pain, infection, perfusion and systemic problems
  • Categorizing the wound type
  • Develop an appropriate wound care plan
  • Select wound care supplies
  • Clean + prep the wound
  • Dress the wound
  • Leave an infection prevention plan

Following a Wound Care Plan (LPN or RN)

  • Select wound care supplies
  • Clean + prep the wound
  • Dress the wound
