What navigation support looks like

A coordinator who knows the local system sits down with your situation and works out what is actually available to you. That means getting the right assessment started, understanding what is publicly funded and what is not, knowing which waitlist to join and when, and making sure the paperwork does not stall in the wrong inbox.

When a hospital discharge is coming, we help you plan for it before the date arrives rather than after. When a decision needs to be made about staying home or moving to a residence, we lay out the real options and what each one costs.

Why we do this for free

Because a family that understands its options makes better decisions, and because the system is genuinely difficult to use. Navigation guidance is free. Where you need someone physically present, such as attending appointments or dedicated hands-on time, that is provided by our caregivers at standard caregiving rates.
